How to enter

1. Fill out the survey below: this portion is anonymous- no names or contact info are required.
2. Submit the Survey: once you submit the completed survey, you will be redirected to the optional giveaway entry page.
3. Enter to Win: to enter the giveaway, you will be asked to submit your first name and email address. These are necessary to give us a way to contact you if you win and your entry is NOT connected to the survey form.
4. Wait: once you have entered (one entry per person please) the winner will be contacted 2-5 days after the close of the giveaway which will be the beginning of February.
5. Reply: if you are the winner, you will be emailed using the address you supplied (make sure to check for typos). You must respond within 48 hours of receiving the announcement to claim your reward. If you do not respond, you are no longer eligible to win and we will select another winner.
